An affidavit is a sworn statement of facts, or in other words, swearing that the information in the affidavit is accurate and true. We routinely draft affidavits for our clients for a variety of purposes. Examples of commonly drafted affidavits include:
Confirmation of marital status for OSAP or the Ontario Pension Board
Affidavit of Execution for a Will
Affidavit for one and the same/Affidavit of name change (used if you have assumed a new name, or if the spelling or full name on your identification differs from that which you customarily use)
